The Wellness Collaborative Healthy Home Program provides services to low-income, senior renters to help address chronic disease management with the goal of minimizing falls, nutritional risks, lowering Medicaid costs, reducing hospital visits, and enabling seniors to remain in their homes.
Oral Health Care and Assessment and Coordination Project, providing an oral health kit for each of the five Foundation counties, including toothbrushes, fluoride mouth rinse, and educational materials and services for children in the county elementary schools
Training, development, and supervision of Center staff The Center, which provides a safe, child-friendly facility where child protection, criminal justice, and child treatment professionals work together to investigate abuse, hold offenders accountable, and help children heal.
